Consent form templatesfor beauty and wellness services
Three forms cover most services: a health questionnaire, a treatment consent for the specific service and a photo consent. Below are the health questionnaire and the photo consent in full; the treatment consent depends on your specialty.
Health questionnaire and photo consent
Used for every specialty. Copy them as they are, or adapt them for your studio.
Health questionnaire
So we can treat you safely, we ask about your health before your appointment: medical conditions, allergies and sensitivities, medications, pregnancy or breastfeeding, skin conditions, and previous treatments and how you reacted to them.
We use this information only to plan and perform your treatment. We do not sell it or share it with third parties for their own purposes. We keep it for three years and then delete it.
Please tell us if anything changes — including between appointments.
You may withdraw this consent at any time for the future. Treatments that have already taken place and their records are not affected, because we keep them as documentation.
Treatment photos
We take photos of the treated area before and after your treatment. They are part of your treatment record: they show the starting point, make results comparable and may be the only evidence of how your skin looked before.
The photos are stored privately with your client record. Only our team can see them; they are not published and not shared with third parties. They are deleted together with your record after three years.
Before we use any photo publicly — on our website, on social media or in the studio — we will ask you separately and explicitly. Without that additional permission, we will not use it.
You may withdraw this consent at any time for the future.
Treatment consent by specialty
Three forms, three jobs
The health questionnaire finds the reasons not to treat before the appointment. The treatment consent explains the service — how it works, possible reactions, aftercare. The photo consent separates private treatment photos from public use, which always needs its own permission.
These are the same texts clients sign in Luvira, where each signature is stored with the exact wording and version number. In Luvira the wording is fixed today — you cannot edit it in the app yet. Copy it here if you need to adapt it for paper or another system.
Templates, not legal advice. Consent and record-keeping rules differ by state and by service; have the wording reviewed before you use it. Luvira makes no HIPAA claim.

Frequently asked questions
About intake and consent forms
The intake form (health questionnaire) collects information about the client. The consent form documents that the client understood the treatment and agreed to it. Most services need both.
A signed consent documents what was explained; whether it holds up depends on your state and the situation. Treat these as templates and have them reviewed.
Luvira keeps signed forms for three years. Your state board, health department or insurer may expect a different period.
When you copy the text, yes. Inside Luvira the forms use fixed wording for now, without custom questions.
