Intake and consent formssigned before the treatment
For lash extensions, peels, permanent makeup or color, a signed consent is part of doing the work properly. Luvira includes three kinds of forms written for US studios — a health questionnaire, a treatment consent for your specialty and a photo consent. Clients sign digitally, and Luvira stores the exact wording and version they signed.

Three forms, each with a job
The health questionnaire asks about medical conditions, allergies, medications, pregnancy, skin conditions and previous treatments. The treatment consent explains your specific service: how it works, possible reactions, when you won’t treat and aftercare. The photo consent asks whether you may keep treatment photos in the record — and says that any public use needs a separate, explicit permission.
- Lash service consent
- Skin care and aesthetic treatment consent
- Nail, color, massage and permanent makeup consent
Why the version matters
A consent form is a record of what was explained. If you change your form next year, the clients who signed this year signed the old wording. Luvira stores the wording and version number with every signature, so you can always show exactly what a client agreed to.
What it doesn’t do
- Templates, not legal advice — review them for your state and services.
- No HIPAA claim; Luvira is not an electronic health record.
- The template wording is fixed in Luvira today — you cannot edit it in the app yet. If your state needs different wording, keep using your own form for that service.
Consent templates are general templates, not legal or medical advice. Check them against your state rules and your services before use.
